In November 2003 a SENCOnet meeting was held which looked at good practice in Transition planning.  At this meeting and in previous discussions SENCOs voiced their concern that the format of the Transition Plan pro-forma was neither user friendly nor accessible to all those involved in the process.  Several SENCOs expressed an interest in being part of a reference group that would look at revising the current pro-forma.

A working group was set up to review the LEA Transition Plan pro-forma and to support in devising accompanying guidance for those involved in the process.  The group comprised Julie Thompson – SEN advisory teacher, SENCOs / SEN teachers from KS3 and KS4 schools, Connexions LDD Personal Advisers and SENA.

A pack of information has now been devised to support schools in the transition planning process.  Four files are available you to print and/or download.  These include:
