Assessment: What you can
expect of us
When we are working with you to assess your needs, we will:
- Ask for your input and opinions
- Listen carefully to what you have to say
- Write your views on the assessment form
- Offer advice, and if necessary, provide support to help you
- Involve all organisations (for example, a care agency that may be providing you with help at home) to get their views, if that is what you want
- Keep you up to date with progress and give you the name and contact details of the member of staff that is helping you
If at the end of your assessment you are
eligible for support we will explain what happens next. For longer term support we will explain
the choices you have about how you would like to arrange, buy and manage your support. If you
are not eligible there may be other ways we can help and these will be discussed with you.
