This includes the NHS, other councils, care providers, patients and service users. Joining up health and care in Leicestershire We are working together to provide more joined up care in the community, particularly for frail older people, those with long term conditions and other vulnerable groups, including carers. Health and Wellbeing Board Leicestershire’s Health and Wellbeing Board is made up of local councillors, GPs, health and social care officials and representatives of patients and the public. The board was set up to lead work to improve the health and wellbeing of the population of Leicestershire through improved and joined up services. Better Care Together - a five year planYou may also be interested in the five year plan for local health and care services across Leicester, Leicestershire and Rutland. This is know as the 'sustainability and transformation plan' or STP. Each local area must have a plan to show how services will evolve and become sustainable to support the NHS vision for better health and patient care.
