Short breaks for carers

The social care assessment determines both eligibility and any financial contribution required. 

To begin, you (or a family member on your behalf) can request a:

• Self-assessment for care and support needs 

or a

• Carers assessment

Once approved: 

• a location that suits your needs will be allocated to you
• you can request the dates you'd like to use the service

If your request can be accommodated, you’ll be invited to visit the service location. 

This process makes sure that support is tailored, accessible, and aligned with your personal circumstances.
