The Better Care Fund (BCF) integrates health and social care services to help people live independently at home and reduce hospital admissions. It achieves this through a pooled budget from the NHS and local authorities, supporting initiatives like hospital discharge, reablement, and prevention.
  
We work with the Integrated Care Board (ICB), NHS partners and other local authority partners to deliver this on behalf of the Health and Wellbeing Board, bringing together health and social care services to support people in their communities through joined-up, preventative care.  

Key initiatives include neighbourhood model of care, population health management, virtual wards, and care technology, all aimed at improving independence, recovery, and wellbeing. Services also focus on falls prevention, integrated personal care, intermediate care and specialist support for neurological conditions.
