Integrated Care Service referrals from your doctor

This service creates a link between health, social care, and community services to provide individuals and families with a seamless experience.

The Integrated Care Service

Most GP practices across the county have an Integrated Care Co-ordinator who is there to help make sure you receive the right support, at the right time, to remain independent and well.

Once you have been referred by your GP to the service, a Care Co-ordinator will contact you directly to discuss what matters most for your health and wellbeing and how they can help you manage daily life and stay independent for longer.

They will work with the health professionals, other partners and voluntary organisations involved in your care and can arrange referrals, equipment, or support on your behalf.

Access to the service

If you are an older adult living independently at home, have complex or long-term health conditions or feel that you would benefit from extra support, speak to your GP and ask if you can be referred to the Integrated Care Service.

Key information

  • If your GP refers you, you don’t have to take part. The service is optional, and you can opt out at any time.
  • The service doesn’t replace your GP or nurse.  Your usual care continues as normal.
  • The service is free. If adult social care support is needed, a financial assessment may be required
  • Your personal information is kept safe and only shared with services you agree to involve
