Our Home Care Assessment and Reablement Team (HART)
We support people to live independently in their own home, often helping them to return home from hospital or preventing them from being admitted to hospital.
We also provide personal care for adults in their own homes in crisis situations. For example, after a hospital visit, we support adults on a short-term basis whilst longer term care is arranged.
Each year we provide short-term support to around 4,000 adults in Leicestershire and aim to help them to live as independently as possible and to assess their long-term care needs.

We are looking for enthusiastic and caring individuals to support adults who have a range of care needs in Leicestershire by helping them with everyday tasks, be active in their community, and safeguarding them from harm.

About the HART roles
The roles are multi-faceted and provide support for people in their own homes throughout Leicestershire. We offer on-the-job training that will help you develop your skills and confidence while you work.
Duties often include:
- Personal care such as washing and dressing
- Preparing food and drink
- Help moving around at home
The people we support may have learning disabilities, physical disabilities, dementia, and other mental health conditions.
You will work with other care professionals to meet identified goals and support needs.
Skills and qualifications
Within a reablement role, it is important that you have:
- a kind and caring personality and a professional attitude
- good English, numeracy and writing skills
- basic IT skills to update and maintain records
- an ability to understand and follow procedures
- good communication and listening skills
- an ability to maintain confidentiality
- a driving licence and access to a vehicle to travel across Leicestershire
A Level 2 / 3 Diploma in Health and Social Care (or a relevant equivalent qualification) is desirable, but not essential as training can be provided to work towards this qualification once you start the job.

Work life balance
Work life balance is important to us and that is why we have part-time and full-time opportunities available, ranging from 10-42 hours per week.
All roles are on a shift/rota basis and include evening and weekend work. Rotas are organised well in advance, with two non-working days per week and alternate weekends off work.
Training and progression
Once you start the job, you will undergo basic training such as health and safety, first aid, and moving and handling. You may also receive specific training based on what the person you are caring for needs.
You will have every opportunity to develop in the role through:
- regular training and mentoring
- support and advice, encouragement, and feedback
- opportunities to progress and develop with us and take on more responsibility or specialise in a certain area
