Information to bring to your appointment

To register the birth you will need to bring certain information with you.

About the child:

  • date and place of birth - if the birth is one of twins, triplets, etc. the time of each child's birth will also be needed.
  • sex of the child
  • the first name(s) and surname the child will use growing up

About the mother:

  • first name(s) and surname, and any previous names you’ve used
  • maiden surname - if you are, or have been married or in a civil partnership
  • date and place of your birth
  • job at the time of the child's birth or, if  unemployed at that time, details of your last job
  • address at the date of the birth
  • date of marriage or civil partnership, if married or in a civil partnership with the child's father at the time of the birth
  • number of previous children born to this mother

About the father or second female parent:

  • first name(s) and surname, and any previous names you’ve used
  • date and place of your birth
  • job at the time of the child's birth or, if unemployed at that time, details of your last job
  • address at the time of the registration of the birth

It’s important that the information recorded in the birth register is correct. Any mistakes made, for example in the spelling of a name or in the description of an occupation can be difficult to put right after you’ve signed the register. You should check the details very carefully before you sign.
